Dear Sir,

 Please inform me when I can attend your facility for a medical check. I need to provide a certificate that I am fit to work.

 I am travelling to Italy on business from Monday next week until the Friday.

 I am English and have worked in Poland for 8 years, but only the last 2 I have been employed by a Polish Company.

 I live in  on the outskirts of Wroclaw so I can drop by before, after or during work Best Regards
